The Objective
To illustrate sustained improvement in the management of hypertension across a wide range of patients.
Inclusion Criteria
This study includes patients with a hypertension diagnosis (the qualifying criterion for RPM in this case) whose average systolic blood pressure in their first month with the program or “Month Zero” was greater than or equal to 150.
Considerations
This study includes patients who are still active with varying durations in the program. This study also includes patients who have since deactivated after varying durations in the program.

Outcomes & Findings
- On the whole, patients’ average systolic blood pressure trends downward, regardless of practice.
- There is an undeniable positive correlation between patients who stick with the program and improved BP as compared to those within their practice cohort who leave the program
- Patient-level drilldowns empower clinicians to commend those who are improving while focusing on changes for those who aren’t
- Affecting change in wellness and engagement
- Once introduced to the program and activated, patient behavior patterns changed
- The takeaway: patients undeniably improved their blood pressure once given a device and encouraged by nurse coaches to use them
- Empirical clinical data pointing to quality improvement now exists for practices
- For just 4,004 patients in this study, we’ve captured over 900k BP readings that would otherwise not be available
- In the same timeframe, their have been almost 700k unique patient days where a patient engaged in their wellness while sitting down to take a reading
