Rural Health Clinics

Patients in rural areas are often underserved and face logistical challenges accessing care. With the changes to General Care Management Code G0511, which includes RPM and CCM, Federally Qualified Health Centers (FQHCs) and Rural Health Clinics (RHCs) can expand their patient monitoring capabilities.

RCP is perfectly positioned to help you set up the right program, regardless of your patients' proximity to the office.

Senior Communities

RCP provides a comprehensive toolkit to medical groups and senior living facilities – allowing you to track the health of your patient population in real time. We provide the technology and data to identify which patients need your attention and make it easy to connect with them, without relying on patients to reach out or schedule appointments. As the senior population continues growing, RCP's comprehensive toolkit empowers practices to proactively manage patient health, including medications, care plans, sleep, steps and device data.

Home Health Agencies

RCP offers a comprehensive solution for home healthcare agencies aiming to enhance patient care and satisfaction. Our services are designed to improve quality scores and elevate your agency's standing with referral sources. By partnering with RCP, agencies can access a range of Medicare-approved services that utilize our cutting-edge hardware, software, and oversight staff, all without altering the patient's primary care physician. This collaboration ensures proven reductions in hospitalizations and emergency room visits, ultimately lowering healthcare costs. Additionally, our platform is attractive to both patients and their families, who can conveniently access data through our app. For home health agencies, this means enhanced care during the service period, a sustained relationship with patients, and improved quality scores and patient outcomes.

Frequently Asked Questions

from providers

I've heard CCM and RPM aren't financially attractive. Can I lose money?

The short answer is that CCM and RPM are almost always financially attractive for medical practices. While there are some pressures around reimbursement rates, copay collections, and patient adherence to program rules (note: other services have the same pressures), RCP's RPM program is designed to be cash-flow positive from the start, even with small initial patient populations. We recommend asking RCP for a detailed financial forecast to understand the potential revenue and costs for your specific practice.

What's the patient acceptance rate?

RCP has achieved nearly 50% patient penetration at our best-performing practices. When patients are asked by their provider or staff to participate, the acceptance rate is even higher.

How do I enroll patients?

We focus on connecting with patients around their in-office appointments, with some mailings and outreach to all eligible patients not already enrolled once or twice per calendar year.

What's the expected attrition rate?

Approximately 5% of patients will leave the program within the first 3 months, deciding they do not need or wish to participate. After that, the attrition rate is around 2% per month due to factors like moves to senior living facilities or outside the physician's service area.

What kind of reporting do I receive?

Reporting is a significant asset of RCP's program, with a 20+ page monthly report that includes a curated list of patients needing attention or positive feedback. Clinically, the patients needing attention are most important, but sending a "good work" message to a patient is important for engagement and satisfaction.

How does my staff work with RCP?

Each partner practice has an account executive for business and operational issues. The nurse coaches who work directly with your patients have a close relationship with your team, often communicating via EMR messaging, phone calls, and even conference calls with the patient when needed.

What are the benefits beyond revenue?

Many practices don't realize that RCP effectively adds a full-time staff member for every 200 patients enrolled, often taking on the most challenging patients in day-to-day practice operations. Additionally, patient satisfaction is significantly higher with RCP's program.
