A personal Nurse Coach will reach out if there is a need to discuss readings, offering insights that will help patients stay connected and in control of their healthy lifestyle.

HeathyConnect is often free for most Medicare patients or for just a minor co-pay

HealthyConnect is a whole new care model.  
If patients are a member of a Medicare Advantage plan, there may be a minor co-pay required.
